HPH Hospice Receives Sam’s Club Grant in Support of Camp REACH OUT

Brooksville, FL (May 15, 2019): HPH Hospice received a $500.00 grant from Sam’s Club, store #4818 in Brookville to support Camp REACH OUT.

Camp REACH OUT is an annual weekend camp for children living in Pasco, Hernando and Citrus Counties, ages 5 to 17, who experienced the death of a loved one.

Chapters Health System Recognized Again by the Great Place to Work® Institute

Tampa, FL (April 11, 2019): Chapters Health System was certified for the second year in a row as a great workplace in the Aging Services category today by the independent analysts at the Great Place to Work® Institute. Chapters Health and its affiliates (Good Shepherd Hospice, HPH Hospice, LifePath Hospice, Chapters Health Palliative Care and HPH Home Health) earned this distinction based on extensive ratings provided by its team members in anonymous surveys. LifePath Hospice to Host 27th Annual Camp Circle of Love

TAMPA, FL (March 18, 2019): The weekend of March 22 through 24, 2019 marks the 27th annual Camp Circle of Love, hosted by LifePath Hospice. Children ages 6 to 18, who live in Hillsborough County and experienced the death of a loved one, attend the camp that emphasizes the development and building of trust, coping with and managing grief. Each year the camp takes place at the Lakewood Retreat in Brooksville. To date, nearly 2,500 campers have been helped through Camp Circle of Love.


Chapters Health System Partners with WellSky to Enhance Post-Acute Care Across the Continuum

LENEXA, Kan. — March 12, 2019 — WellSky, a leading health and community care technology company backed by TPG Capital, today announced a partnership with Chapters Health System to deploy and develop its web-based electronic health record (EHR) technology solutions. Chapters Health and its affiliates will implement hospice EHR and palliative care software from Consolo, a WellSky company, as well as WellSky Home Health software to enhance the continuum of post-acute patient care services provided across eight Florida counties.


Chapters Health Foundation Presents 7th Annual One Night of Gospel Event

TAMPA, FL (February 27, 2019): Presented by the Chapters Health Foundation, One Night of Gospel has been a welcome community event for the last six years. This year’s concert will take place at the Church at the Mall on Sunday, March 17, 2019 with doors opening at 5:30 p.m. for VIP seating. The benefit concert features Ernie Haase & Signature Sound with special musical guests, the Crossfire Quartet and begins at 6 p.m. One Night of Gospel supports the work of Good Shepherd Hospice and kidsPACK.


Chapters Health System Expands Resources to Provide Home Care in Seven Counties

TAMPA, FL (January 8, 2019): Chapters Health System (Chapters Health) announced the signing of a finalized purchase agreement for Achieve Home Care, LLC (Achieve), which expands its resources to provide home care in seven counties. Chapters Health is a not-for-profit organization headquartered in Temple Terrace, with current affiliates that include HPH Home Health in Pasco and Pinellas counties; Chapters Health Palliative Care in Hillsborough, Highlands and Polk counties; Good Shepherd Hospice in Polk, Highlands and Hardee counties; HPH Hospice in Pasco, Hernando and Citrus counties and LifePath Hospice in Hillsborough County. Achieve currently serves patients in Hillsborough and Polk counties, but is also licensed to provide home care in Highlands, Hardee and Manatee counties.  The integration of Achieve staff and patients will occur over the next three months to ensure continuity of care.


Good Shepherd Hospice Hosts Lights of Remembrance Events

TAMPA, FL (November  13, 2018): On three separate dates, Good Shepherd Hospice will host “Lights of Remembrance,” which are annual commemorative ceremonies for families and community members to acknowledge and appreciate cherished loved ones. The holiday memorial ceremonies are scheduled for Tuesday, November 27; Tuesday, December 4 and Thursday, December 6. Starting at 6 p.m., each holiday memorial ceremony, which is free and open to the public, will take place at several locations throughout Polk and Highlands counties.


Chapters Health System Picks Dean H. Forman, MBA as Its New COO

TAMPA, FL (November 8, 2018): Chapters Health System and its affiliates (Good Shepherd Hospice, HPH Hospice and LifePath Hospice) announced today  Dean H. Forman, MBA, will start officially as the new Chief Operating Officer on Monday, December 3, 2018. Forman will report directly to President and Chief Executive Officer Andrew Molosky. In 2017, Chapters Health affiliates served almost 19,000 patients and families across eight counties—Citrus, Hardee, Hernando, Hillsborough, Highlands, Pasco, Pinellas and Polk.
