Care Connect

One of the hardest decisions a person with a terminal illness can face is the decision to stop curative treatment. What if there was another option? Through Chapters Health Care Connect, the Centers for Medicare and Medicaid Services (CMS) provides a new option for select Medicare beneficiaries to receive supportive care services typically offered by hospice. Patients can still receive curative treatments from their care team.

This new model is part of a nationwide initiative created to deliver better care and put patients at the center of their care. LifePath Hospice, an affiliate of Chapters Health System, was selected as the only participating organization in Hillsborough County to offer these services through its Care Connect program.

On Wednesday, September 11, 2019 from 2 to 2:20 p.m. (EST), CMS will be presenting a special event for community providers. This 20-minute webinar session will feature an overview of the eligibility criteria for the program and showcase currently engaged healthcare providers about why they refer their patients to the program. Care Connect Eligibility

If patients are interested in receiving Care Connect services, they must meet certain criteria, which we are happy to discuss. Only individuals who have been diagnosed with advanced cancer, congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD) or HIV/AIDS are eligible to participate in the Care Connect program. However, if patients don’t meet the criteria for Care Connect, Chapters Health offers other options to help them make the most of life.

Under Care Connect, the primary care physician will continue to be in charge of care. Our Care Connect team will work together with the physician to provide supportive care services, helping the individual live life on his or her connect

Care Connect Services

Care Connect’s supportive care and services are provided by an interdisciplinary team of nurses, home health aides, social workers, chaplains and volunteers. Services are available by phone — 24 hours a day, 7 days a week, 365 days a year.

Care Connect covers the following services for patients living in Hillsborough County:

  • Care coordination, case management and shared decision making
  • Pain and symptom management
  • Social work services
  • Spiritual care
  • Volunteer support
