Our team members provide compassionate, patient and family-centered services in hospice, palliative care and home health in a variety of settings such as hospice house, hospital, patient homes, nursing homes and assisted living facilities.

The range of care settings and career choices is vast at Chapters Health and provides endless opportunities for those who want to put the patient and families first. We’re dedicated to delivering the best care possible in a family’s time of need.

Nursing

As an RN, you’ll be key in providing top notch palliative and supportive care to our patients and their families. Beyond the opportunity to put the patient needs first, being an RN on our team means enjoying support for ongoing education, certification and tuition reimbursement.

Nursing Leadership

Nursing leaders in our clinical team leader and regional director roles provide leadership and team member support, direction, and guidance to our Interdisciplinary teams. In addition, our nursing leaders are also experts in professional nursing practice in our service lines, clinical operations, care management and patient experience.

Nursing Support

Our LPNs are responsible for the observation of patient and family needs, as well as providing palliative and hospice care to the patient and family unit. In addition, LPNs utilize nursing interventions and teaching skills to maximize patient comfort and enhance quality of life our patient and their family.

Our hospice aides/CNAs performance play a critically important role, providing personal care and homemaker care for our patients and their families. Our clinical ladder program allows our CNAs/hospice aides to empower them to reach their professional goals.

Spiritual Services

Our social workers and bereavement specialists play a vital role in coordinating a wide range of social and grief services, including psychosocial support to patients and their families, as well as emotional support in times of crisis.

Chaplains provide a wide range of spiritual services to patients and families in any of our hospice and palliative care services. Our chaplains also provide spiritual support to our team members and volunteers at Chapters Health.

Advance Practice Nursing

APRNs play a key role in how we deliver care to our patients and families. Our APRNs use their clinical expertise to maximize the quality of life for our patients and their families. Partnered closely with our physicians and nurse leaders, their advanced clinical training allows them to provide palliative and hospice care services to our communities.
