Chapters Health System Expands Resources to Provide Home Care in Seven Counties
TAMPA, FL (January 8, 2019): Chapters Health System (Chapters Health) announced the signing of a finalized purchase agreement for Achieve Home Care, LLC (Achieve), which expands its resources to provide home care in seven counties. Chapters Health is a not-for-profit organization headquartered in Temple Terrace, with current affiliates that include HPH Home Health in Pasco and Pinellas counties; Chapters Health Palliative Care in Hillsborough, Highlands and Polk counties; Good Shepherd Hospice in Polk, Highlands and Hardee counties; HPH Hospice in Pasco, Hernando and Citrus counties and LifePath Hospice in Hillsborough County. Achieve currently serves patients in Hillsborough and Polk counties, but is also licensed to provide home care in Highlands, Hardee and Manatee counties. The integration of Achieve staff and patients will occur over the next three months to ensure continuity of care.
LifePath Hospice Hosts 11th Annual Lights of Remembrance Event
TAMPA, FL (November 27, 2018): Offering an affirmative way to acknowledge and appreciate cherished loved ones, LifePath Hospice will host its 11th annual “Lights of Remembrance” event on Thursday, December 13, 2018. Starting at 6 p.m., the holiday ceremony, which is free and open to the public, will take place at several locations throughout Hillsborough County.
Good Shepherd Hospice Hosts Lights of Remembrance Events
TAMPA, FL (November 13, 2018): On three separate dates, Good Shepherd Hospice will host “Lights of Remembrance,” which are annual commemorative ceremonies for families and community members to acknowledge and appreciate cherished loved ones. The holiday memorial ceremonies are scheduled for Tuesday, November 27; Tuesday, December 4 and Thursday, December 6. Starting at 6 p.m., each holiday memorial ceremony, which is free and open to the public, will take place at several locations throughout Polk and Highlands counties.
Chapters Health System Picks Dean H. Forman, MBA as Its New COO
TAMPA, FL (November 8, 2018): Chapters Health System and its affiliates (Good Shepherd Hospice, HPH Hospice and LifePath Hospice) announced today Dean H. Forman, MBA, will start officially as the new Chief Operating Officer on Monday, December 3, 2018. Forman will report directly to President and Chief Executive Officer Andrew Molosky. In 2017, Chapters Health affiliates served almost 19,000 patients and families across eight counties—Citrus, Hardee, Hernando, Hillsborough, Highlands, Pasco, Pinellas and Polk.
LifePath Hospice Opening New Inpatient Hospice Unit at Tampa General Hospital
TAMPA, FL (September 21, 2018): Located on the fourth floor in the East Pavilion at Tampa General Hospital (TGH), LifePath Hospice will open its new inpatient hospice unit on Monday, October 1. The 8,500 square-foot unit featuring 16 private rooms is the first of its kind in Hillsborough County.
Good Shepherd Hospice Philanthropy Group to Hold 3rd Annual Seasons of Service Luncheon
TAMPA, FL (September 20, 2018): The Hospice Women of Giving, a philanthropic group that supports Good Shepherd Hospice, will hold its third annual “Seasons of Service” luncheon featuring a panel of community women on Thursday, October 4, 2018. HPH Hospice Seeks Prospective Volunteers
