The Chapters Health Nursing Home Diversion program is a Medicaid Long-Term Diversion Program. Our program provides a wealth of benefits and services to meet seniors’ needs — at home. It includes:
- Personal care – assistance with eating, bathing, dressing, personal hygiene and other activities of daily living
- Occupational, physical and speech therapy at home
- Nutritional and dietary assessment – evaluation and instruction on nutritionally appropriate meals
- Home-delivered meals – nutritious meals delivered to the home
- Personal emergency response system – electronic device to secure help in an emergency
- Family training – instruction on treatment regimens and use of equipment
- Consumable medical supplies – incontinent supplies and nutritional supplement
- Adult companion – nonmedical, supervision and socialization
- Case management – evaluation and coordination of service
- Chore services – help maintaining a clean, sanitary and safe living environment
- Transportation – to and from service providers
- Adult day care – caregiver support and respite care in an active and socialized setting
- Assisted living facilities – provides 24-hour supervision, assistance, socialized peer interaction, dining and housekeeping service
- Skilled nursing – services offered in a healthcare facility
