Honor Loved Ones with Memorial Gifts
Family members and friends often ask how they can give back or make memorial gifts to the hospices of Chapters Health System to show their appreciation for the compassionate care provided to them and their loved ones. We are deeply grateful for such gestures, which help ensure that we can serve patients and families who otherwise cannot afford hospice care. Many families ask that charitable gifts be made in lieu of flowers. If you would like to direct tribute gifts to a hospice of Chapters Health—Good Shepherd Hospice, HPH Hospice and LifePath Hospice, or if you would like to make a gift to one of our hospice houses, they are as follows:
- Lakeland Hospice House in Lakeland, George C. Forsythe Hospice House in Auburndale and Bud & Donna Somers Hospice House in Sebring (Good Shepherd Hospice)
- Marliere Hospice House in New Port Richey and Sturgill Hospice House in Brooksville (HPH Hospice)

When a memorial gift is made in the name of a loved one, each individual donor is sent an acknowledgment letter. In addition, the family will receive a prompt notification of the gift so that they can personally thank the individual making the memorial contribution.
Whatever form your gift to Chapters Health takes, it is a gift of caring. Your generosity and compassion will help Chapters Health continue to provide programs and services to those in need. As a donor, you will have the satisfaction of knowing that you have made a difference in the lives of people living in our community.
